You have the right to: inspect and copy your health information; request an amendment to your health record; request restrictions on certain uses and disclosures; receive confidential communications; receive an accounting of disclosures; and receive a paper copy of this notice upon request.
Our Duties
We are required by law to maintain the privacy of your health information, to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of the notice currently in effect.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our office or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
