New Patient Forms
Please download, complete, and bring the following forms to your first appointment. Arriving with completed paperwork helps us dedicate your full visit time to your care.
Forms are available at the front desk if you prefer to complete them in person. Please arrive 20 minutes early if doing so.
Fees
We are a fee-for-service practice. We do not bill insurance directly, but we are happy to provide a detailed superbill you can submit to your insurance company for potential reimbursement.
Initial Consultation
90-minute comprehensive intake — $250
Manipulation Visit
20 minutes — $150
Lab Review & Interpretation
30 minutes — $95
Superbill Provided
We provide itemized receipts for insurance reimbursement submission.
HSA / FSA Accepted
Health Savings Account and Flexible Spending Account cards are accepted.

What to Expect at Your Visit
Before Your Appointment
Complete your intake forms and gather any recent lab work, imaging reports, or specialist notes. The more context you bring, the more productive your visit will be.
Your First Visit
Your initial consultation is 90 minutes. Your practitioner will review your full health history, discuss your goals, and perform any relevant assessments. No rushing — this is your time.
Your Care Plan
At the end of your visit, you'll receive a personalized care plan outlining recommended therapies, lifestyle adjustments, and any follow-up testing.
Ongoing Support
Follow-up visits track your progress and refine your plan. We're your partners in health — not just for one visit, but for the long journey.

Telehealth Policy
At this time, Convergence Medical Center does not offer telehealth or virtual appointments. We believe that in-person care allows us to provide the thorough, hands-on assessment our patients deserve. All consultations, follow-ups, and lab reviews are conducted at our office.
